Application Procedure

  1. Obtain survey from designated school counselor, our Web site or an Upward Bound staff member
  2. Return survey to designated school counselor or mail/fax to Upward Bound Office

  3. After completed survey is received it will be reviewed by the Upward Bound staff
  4. If eligible according to survey, the potential participant will be contacted by the Upward Bound Office and sent an application packet.  Applicant also will be asked to schedule an interview
    • The following must be present at the interview to complete the interview portion:
      • Potential participant
      • Parent or legal guardian of potential participant
      • Potential participant's most recent report card
      • Medical insurance card for potential participant  
      • Completed Application (if you did not receive an application prior to interview, you will be ask to complete application during the interview process)
  5. Upward Bound staff will complete paperwork and forward information to the director of the program, Ms. Barbara Berry.  She will review the packet and recommendations of the staff to make her decision.
  6. The potential participant will be contacted regarding their status
