Computer Output Request Form

Lists with information queried from Colleague can be requested by using this online form. Requests are emailed to the IT Dept. and assigned to a programmer who will contact you.

Labels are no longer available from IT. Downloaded data files can be created and sent electronically to Printing Services for printing directly on your media, or returned to you for generating labels on your office laser printer.

NOTE: Some requests for student data may need authorization from the Registrar, Dean of Students, or HR.
